This release form is to state, I am 18 years of age or have signed permission below of my parent or legal guardian and I accept full responsibility for the administration and willing participation in the act of receiving a Body Art Tattoo by an Artists provided by INKED UNION, wherein the Tattoo as described in release form will be placed on my body on the specific area listed in this form. I furthermore release and give up… All Rights to any legal action taken against Artists Producing Body Art Tattoo and INKED UNION as a Company. I Acknowledge that I have freely and voluntarily chosen the Tattoo received and the location of it.
I DO NOT TO MY KNOWLEDGE, HAVE ANY OF THE FOLLOWING DISEASES, AILMENTS OR VIRUSES:
Corona Virus / COVID-19, Diabetes, Hepatitis, Herpes, Acquired Immune Deficiency Syndrome (AIDS) or any other Communicable Diseases or infections.
